Assisted Living Facilities

The Assisted Living Facilities Comprise of
Three Components That Sustain Them In The Market.

Real estate

All three of them play an equal part in the success of the facilities. Even though wellness is very important for the wellbeing and longevity of the residents, the facilities are struggling as there is a significant knowledge gap of what it takes to achieve and lack of understanding how this alone can improve their bottom line.

Our focus is to help the Facilities improve the Wellness of their residents by addressing 7 major Wellness issues that facilities handle every day

01Lack of integrated Care - Declining health, medication management and care coordination are the leading reasons residents come to assisted living. It’s a drainer on the assisted living care staff because its time consuming and staff dependent.

02Transitions of care - From hospitals, Rehabs, Physician visits. It involves significant time, is detail driven, involves multiple third-parties and requires extensive care coordination.

03Increasing acuity - Needs attention to detail and an interdisciplinary approach.

04Chronic Disease Management – Average AL resident has a minimum of 2 chronic conditions that impact their outcome and life span.

05Recurrent falls.

06Recurrent Urinary infections.

07Neuropsychiatry issues.

We Address These Through 2 of Our Key Products, ACW and ACM

Assisted Living
Care-Coordination & Wellness
(ACW) - No cost to facility

This is vital for the financial success of the facility for the following reasons:

01One of the biggest reasons the residents are moving out of the facility is because of overall shortcoming in their approach to wellness.

02On the other hand, a properly implemented wellness program can increase each one of your resident’s life on an average by 6 months, resulting in roughly an additional 1.5 million dollars in revenue for every 50 residents.

03Implementing ACW also results in increased operational efficiency.

Automated care -coordination and wellness management (ACM)

Why is it important?

Studies show workforce efficiency in general results in around 80% of the time spent on busy work and 20% of the time spent on productive work. This is where issues arise. The automated care coordination (ACM) takes away the clerical work of wellness coordinators and gives them time to be efficient on matters that matter, and also have more hands-on time with residents rather than being on the phone and faxing. We estimate ACM could increase efficiency of the staff by 30% minimum. Also, with its easy access to providers with in your network, ACM allows tremendous stress reduction.
